The proposed undertaking is a 250-foot (lit) monopole lattice-designed telecommunications tower built to accommodate multiple carriers. The tower project area is located at 2811 U.S. Highway 441 South, in the vicinity of Milledgeville, Baldwin County, Georgia (USGS, Milledgeville Georgia Quadrangle). R.S. Webb & Associates (RSWA) was contracted by Environmental Engineers, Inc. on behalf of Optasite, Inc., a wireless facility applicant. Following please find the Attachments required by Form 620 under the March 7, 2005 Nationwide Programmatic Agreement.
